New Patient Information
Welcome to Women's Care for the Commonwealth. If you are a new patient, please download and print our new patient registration form, fill it out and bring it with you when you visit our office. When you arrive, you will also be asked to fill out a HIPAA (Health Information Privacy and Accountability Act) form that legislation enacted in April of 2003 requires of all patients. Receipts & Appointment Cards
Upon your departure from our office, you will be provided a receipt for services and an appointment card for your next visit.

After Hours Contact
If you need to contact our office after regular office hours, please call the Ephraim McDowell Hospital Operator at (859) 239-1000.

Prescription Information
Please do not call requesting any narcotic pain medication. These medications will not be prescribed after hours or via a phone call.
